These forms are a requirement for all Probus Clubs in Canada, to ensure that the club and its members are properly protected for public liability.

GUEST WAIVER FORM
Guests sometimes join members of the Comox Strathcona Probus Club in an Activity Group activity or special event. Members, please contact the Activity Group leader or event coordinator for permission before bringing a guest.
Some Activity Group activities or events are constrained by the number of participants or other considerations but if participation is possible, guests will be required to sign the Comox Strathcona Probus Club Member and Guest Participation Form (downloadable form below).
Guest and Member both sign this form when a guest participates in a club event. Please complete one form for each guest participating.
Members of other Probus Clubs participating in a Comox Strathcona Probus Club activity or event are already covered by their own club's liability waiver form.
